The Boy Scouts of America (BSA) is a nationwide youth organization that promotes leadership, character development, and outdoor activities for young boys and girls. As part of their program, they require parental consent for a variety of activities. To ensure a safe and enjoyable experience for all participants, all participants must have a completed and signed BSA Permission Slip – Consent Form.

may involve some risk of personal injury and I accept full responsibility for such risks. I hereby
release the Boy Scouts of America and its related entities, their volunteers, service units, and staff,
from any and all liability for any injury or damage which may occur as a result of my child’s
participation in this activity.
